Single Assessment Process Success
In Surrey Heath And Woking Waymarks Route Forward For Other Regions
25 July 2005 — Social and healthcare agencies operating in
the region covered by the Surrey Heath and Woking Primary Care Trust
are now benefiting from the roll-out of a computerised solution to
underpin the Single Assessment Process (SAP). Implemented in partnership
with leading IT provider CIBER UK, practitioners within health and
government organisations are now able to collaborate and share information
more efficiently and enhance the delivery of care to older people
in the area. The success of this project demonstrates the way forward
for the rest of Surrey and other regions seeking to achieve an integrated
Single Assessment Process.
“The Single Assessment Process was introduced into the NHS
National Service Framework to ensure that older people's care needs
are assessed thoroughly and accurately without procedures being needlessly
duplicated by different agencies”, explains Ray Morgan, Executive
Director of Woking Borough Council, the lead authority co-ordinating
the collaboration between multiple partners and CIBER UK as part of
the ODPM funded Fame National Project. “The Local Authority
and Health partners in our region had a vision for realising an effective
Single Assessment Process but we recognised that we couldn’t
achieve it without the effective use of technology to support the
process. Working with CIBER UK, we have brought into place a comprehensive
SAP solution that caters for Social and Health requirements, was easy
to implement and use and, very importantly, is popular with the practitioners
who are conducting the assessments.”
Several key partners are involved in the deployment and use of the
system including Surrey Heath and Woking PCT, Surrey County Council,
Surrey Heath Borough Council, Frimley Park Hospital NHS Trust, Ashford
and St Peters Hospitals NHS Trust and Woking Borough Council.
The CIBER SAP solution links existing agencies and their systems
together to share assessment information. Assessments can be captured
and viewed through a web browser enabling, through a choice of online
and offline options, remote use by practitioners out in the field.
Integration with data stored in existing agency systems, such as Social
Care or GP Practice Systems, also ensures that basic information such
as addresses and dates of birth and, where appropriate, medication
and relevant medical history are accessible and need not be re-entered.
In the case of the SHAW region this involves integration with the
SWIFT social care system from Anite, as well as Vision system for
GP Practices from InPS.
As well as being stored and available locally, completed assessments
are distributed to other agencies through a centralised broker system.
This means that users can only see the information for which they
have permission, according to Information Sharing Protocols, DPA and
patient/client consent rules.
Chris Dunstan, GP, SHAW PCT, Member of the Clinical Advisory Group
and reviewer for NPfIT SAP Best Practice Group, believes that the
solution is making a big difference. “As a GP involved in SAP
development and implementation locally and nationally, the solution
is innovative and practical to use. It helps all of the clinicians
to do their job efficiently by giving them easy access to information
gathered at previous assessments and the ability to generate new assessments
quickly and easily.”
The deployment of the solution by CIBER at SHAW was also selected
as an exemplar project for the FAME 2 programme and Geoff Young, E-Services
Programme Director for Woking Borough Council acknowledges the achievements
that have been made. “All parties involved in the project agree
that the CIBER single assessment technology offers significant efficiency
and service improvement opportunities. In Surrey, we have found the
benefits of the CIBER SAP solution compelling from a business point
of view. On the one hand, practitioners have relevant information
on tap and are able to focus their time with clients to provide a
better service. On the other, the fact that multiple collections and
entry of the same data is avoided means that practitioners can provide
a higher level of care without increasing costs.”
Based upon the success within the SHAW region, Woking Borough Council
and Surrey County Council are committed to working with partners to
roll out the solution across Surrey. As Ray Morgan comments, “Woking
Borough Council is already engaging with neighbouring areas to take
advantage of the CIBER solution and SHAW PCT is working closely with
us in the roll out of the solution across its own area, and helping
other PCT areas participate in a wider roll out across Surrey”
